1. SSM certificate
2. Premise license from the Local Council (PBT)
3. Digital copy of:
   • SSM certificate
   • Spa/PUK license
   • Spa/PUK menu
   • Treatment price list
   • Images of the spa/PUK areas
   • Staff uniform
   • Training manual and SOP (Table of Contents only)

FOR AFFILIATES:
No documents are needed.
