AMSPA Membership Application & Renewal
Before you proceed, here are a few things you should ensure:
FOR ORDINARY AND ASSOCIATE APPLICATION:
Make sure the following documents are readily available (where applicable).
- SSM certificate
- Premise license from the Local Council (PBT)
- Digital copy of:
- SSM certificate
- Spa/PUK license
- Spa/PUK menu
- Treatment price list
- Images of the spa/PUK areas
- Staff uniform
- Training manual and SOP (Table of Contents only)
FOR AFFILIATES:
Digital copy of professional certificates, business registration or recognised qualifications related to the spa and wellness industry (SKM, CIBTAC, CIDESCO, ITEC, etc.)
